The item presents an insertion a two folia in the middle, namely fol. 6 (foliated) and a subsequent blank folio. The content and the handwriting of fol. 6 differs from the rest of the document, and presents a additional instructions for the deacon, arch-deacon and the priest for the ordination.

A few names are recorded in the document, which are linked to the appointment of the new clergy of St George's Church in L'viv. Namely:
